Dear Client,
Purpose of this document
This document is to inform you what data Therapist collects from you and how Therapist will manage and process your data. You are being asked to confirm that you agree with these terms. You can only make any bookings with ‘Free of That’ (Claudia Nawroth) if you agree to these terms.
It is essential for the success of the services Therapist provides to you to collect essential data about you and your circumstances relating to you and the issues you request Therapist to help you with, i. e. which are discussed before and during the therapy.
What data does Therapist collect?
Name and age – this is basic information that helps Therapist to get to know you.
Address, email address, phone number – Therapist uses this as a way of contacting you regarding your sessions and any communication relating to your bookings. Therapist will mainly use your contact data that you provided when you first contacted Therapist to communicate with you. Therapist assumes that you have provided your
contact details correctly and truthfully.
Next of kin/medical professional’s details – If Therapist was worried that you were at risk then Therapist may need to contact your next of kin or medical professional.
Session notes – Therapist keeps brief notes of the Consultation Call and any RTT session(s). Therapist might scan any handwritten notes made relating to the conversations and RTT sessions with you, and the information will be stored securely online in Therapist’s Google Drive for as long as legally required. The folder where your information will be stored is only accessible by Therapist.
Will Therapist share your data and if so, who will Therapist share it with and for what purpose?
Therapist will not sell or use any of your data for unethical reasons or for financial gain. Only in specific circumstances Therapist may have to share your data with 3rd parties, i. e. if Therapist is subpoenaed by court, or if you or anyone you tell Therapist about is at harm or risk of harm.
How will I store your data?
Your data will be stored securely online and in certain cases for a period of no more than two weeks as hard copy in a safe location that is locked (filing cabinet).
How long will Therapist store your data for and how will Therapist dispose of it?
I will keep your details and session notes for as long as any active treatments are continuing, and for the time required by my insurer or for as long as legally required in the country where Therapist’s business is registered. After this time, I will destroy or delete all of your data.
